IMPORTANT – READ BELOW!!

Multi-Club Tryout Form:    If an athlete intends to tryout with multiple clubs, THIS FORM is required to be turned in to each club to let them know. Please bring the signed & completed form to tryouts. If there are any questions regarding trying out for multiple clubs, please visit the Palmetto Region website’s information page.

Medical Release Form:   All athletes are also required to bring a USAV Medical Release Form to all tryouts. This form must be filled out and signed by both players & parents.

USAV Membership Registration:  The first step in the registration process is to obtain or renew a USA Volleyball Membership. Please review our “About Us” section that references the benefits of a USAV Membership and our affiliation with the USAV and our governing body: Palmetto Region

USAV Membership Registration is opened as of September 1st. This membership is required prior to participation in any club tryout
